To start the claims process, we will need the following information:

 

• Policyholder information: Name of insured, address, phone number, e-mail and policy number

• Description of Event: Time and date of loss, location of incident, detailed description of damages/injuries

• Police Report (for traffic accidents only): Copy of any police report when injured

• Itemized Hospital Bill: Please note all authorities included (fire dept., police, ambulance, etc.)

• Claim Form: Contact agent for authorized claim forms

• Additional comments: Is there anything else you think we should know about? (injuries, witnesses, etc.)
